| The following Items need to be submitted to hold a spot in one of our clinics: | ||||||||
| 1) A Waiver. CLICK HERE for Waiver. If the player already completed a waiver for any of our programs this summer then you need not complete another one. | ||||||||
| 2) A Clinic Form. CLICK HERE for Clinic Form. Please fill out the clinic form with all the player's emergency information. | ||||||||
| 3) A check for $20/clinic made payable to Dallas Force Volleyball Club. Please mail this to Dallas Force Volleyball Club, PO Box 3353, McKinney, Texas 75070. | ||||||||
To guarantee a spot in advance, please mail registration to the PO Box. If it is less than a week before the desired clinic, please E-Mail Forrest to put your name on a spot, but understand that if a registration is received in the mail prior to the clinic date, then someone else may get the spot. |
